The actions planned in this Global Challenge are based on four domains of work, one for each fundamental problem identified.
Focus should be rightly on person-centred medication review as a vital part of tackling polypharmacy.
In 2017, a study into the medication usage in older people, including both prescribed medicines and over the counter products, showed that there had been a dramatic increase in use over the last two decades, with a quadrupling in the number of people taking five or more medicines from 12 to 49%.
The average cost saving per patient was £100 per year.
This is an increase of 46.
